Custom Orthotics: Includes postural and gait assessment as well as custom orthotics: $445
*This is a fee-for-service practice. I will not bill your insurance company directly, however you will be provided with an invoice with all applicable codes should you wish to apply for reimbursement. In most cases I will be considered an out-of-network provider. Typical reimbursement rates are from 50-80% of my charges once you have met your deductible. I strongly encourage my patients to research their out-of-network benefits prior to their first appointment (including whether or not they have an out-of-network deductible). Please note: I am not contracted with Medicare or Medicaid, therefore I am unable to see patients who are enrolled in these programs.
Because my focus will be on you and not your insurance company’s requirements, limitations, and billing procedures, you can expect me to go above and beyond what you might expect in a typical insurance-driven physical therapy environment. You can expect to make significant progress quickly and have a very clear understanding of what your problem is and what needs to be done to correct it.
**Payment at the time of service. Checks, cash, credit, and Paypal accepted. Also, you will likely be able to use your Health Savings Account (HSA) or Flexible Spending Account (FSA) as payment.
***Please read…Cancellation Policy: Because I often have a waiting list, 24-hour notification is required for all cancellations so that attempts can be made to fill your vacated spot on my schedule. Patients with cancellations of less than 24-hour notification will be charged the full amount if that appointment time cannot be filled. No Show appointments without prior notification will also be charged the full amount of your visit (less supply charges (i.e. orthotics)). If a phone call is still possible upon no-showing or late-cancelling I am always happy to speak to you. After all, you will still be paying for that time. Note: we cannot book another appointment with one another until this fee is paid.
